Yeah, this one's definitely easy to get confused in. The biggest thing to clear up is what happened immediately before this movie. This takes place immediately after the ending of Movie 2, with Skiki being rushed to the hospital after getting hit by a car (the very final moments of that movie show that happening). This puts her into a 2 year-long coma in which she not only loses her second personality (the male SHIKI) but gains her Mystic Eyes and experiences death. She also loses a portion of her memories and her will to live, which were held by SHIKI. This leaves Ryougi Shiki without the person she felt closest to and without a motivation to keep on leaving, while also having this new power taking over her vision. The point of this movie was to explain her powers a little more, confirm that Shiki did in fact lose her split personality, and set up Araya Souren as the main antagonist and driving force of EVERY movie. He and Kokutou are the only connections between every person in all of these movies, one as a shadowy figure manipulating people and other as a source of comfort or hope for those Souran's manipulating, even without him knowing it. Also, quick clarification since there's still confusion about the puppet. Shiki's arm was a prosthetic in movie 1 because she lost her real arm in the final fight of movie 3. The doll that they keep showing (with the red eyes and similar haircuts) is just a doll that Kokutou felt was similar to Shiki and part of why he has his job. Fun details: You noticed the girl from the first movie in the after-credits scene, but this movie further explains her motives for seeking Kokutou. She briefly mentioned how she was drawn to his commitment towards someone in the hospital she stayed in, showing up to bring new flowers for two whole years. She wanted him because she wanted someone to care for her that much. While Kokutou himself hasn't done much in the plot, he's perhaps the single most important character in this show. As Shiki said, there's one thing that won't disappear. He doesn't have any magic or unique abilities, he's just a very average person, but everyone that we've met so far has come to rely on him. For most of them he represents stability and security, especially for a lot of these very vulnerable people. The obvious examples are Shiki and the two girls she's had to fight so far, but we even saw in movie 2 how one of the men who raped Asagami turned to Kokutou for help. As much as Kokutou personally despised him and said he'd turn him in to face justice, he still promised to protect him from Asagami's wrath and hid him temporarily. For almost every character in these movies he's a source of stability and security, despite his own weaknesses. He's someone who believes in helping the vulnerable, even if it puts himself at risk. This is something that will get challenged repeatedly as the movies progress.

I wonder when will all the characters have secretly meet. The first movie showed the unfinished building with a huge crane at the top, which Fate: Unlimited Blade Works had shown before too.

The Author has another character named Tohno Shiki. Similar to Ryuoji Shiki. But their very slightly different from each other by their rarity (probably).

It be nice to see a world overlapping with each other at the exact same time. KnK, Fate zero, Fate (3 different Shirou's), Tsukihime, Kiritsugu's daughter, the Clock Tower, Illya world and *redacted* world with a different. And many more.

Tohno Shiki is significantly less powerful than this Shiki. But yea it's the male version from another universe.
