This is Fortress of Torm's Hand for the Adventurer's League Epic module : Hellfire Requiem (DDEP09-02). This is the second map of the module.

Torm's Hand is a small village not far from Baldur's Gate, that hosts an important fortress and temple dedicated to Torm.

With some imagination you can also incorporate it into DiA.
