If you checked out the "horror" version of the Asylum, this is just before it happens. The Head Mistress has one of the crystals on her hat, and when you grab it, the scene transforms to the nightmare (not in this release)

 This is just "watch and enjoy", and it ends when she drools... No key required, no auto exit.
