Patreon Feedback Survey (Patreon)

Hey everyone, I made a brief survey to try and get feedback for things here on my patreon. If you have a few minutes I would really appreciate any input! Te fhorm is done through google forms and is completely anonymous. I'm just looking to see if I could maybe improve things around here for you all. 

Note: All open response questions are optional, but multiple choice stuff is required. If you don't feel like typing stuff out, no need, although I would appreciate input in those as well!
