Easily distracted, difficulty thinking things through, etc. Should a physical transformation accompany this?

I'm undecided on this, and thought "hey, I've got people to think FOR me!"

I'd say it depends on the context. If that could be a reasonable part of the spell, or if its's non-canon, sure, give 'm the full package, but if it's supposed to just be mind-affecting in-canon, maybe not necessarily.
