I'll need 3 things off you so we can get started:  

1. A reference of yourself or the person you wish to change  

2. A reference of the character you wish to turn into.

3. A basic outline of what you'd like to see being changed within the 2 panels you'll get each month. For example:

Month 1:

Hair gets longer

Month 2:

Face changes

Month 3:

Breasts grow

Month 4:

Butt expands

Etc
