It's been 40 weeks of Monogatari unless we skipped one, very close to a year indeed. Araragi crying blood is actually a thing that vampires in this series just kinda do, We saw it in Kizu, though given his low vampirism he shouldn't really be doing that in the first place I think so it's still just as terrifying as you imagined.

crying blood is an exaggerated form of showing emotion in fiction, like you have been crying so long and hard it has started bleeding, I think predates anime or manga and is some historical literature thing.
