Something important that sort of went over my head the first time I watched this is that when rudy is suspicious of tenma, hes actually right about johan. When he says serial killers are all liars, he means that johan is indeed just toying with tenma, the same way he toys with other people hes come into contact with- the split personalities were never real. The show didn't make this super clear but that theory is pretty much null and void after this episode
