I remember the Gender Wars when it was announce that Kiku is a guy but a women at heart. There was so many discussions about this topic. Some were mad at Oda for bringing this into his series & other people were happy that he did this.

There were hints of Kiku's secret since she used sessha in couple of her sentences and Kinemon, a massive pervert wasn't that happy when Kiku hugged him and got on top of him probably because he felt something hard

Kawamatsu is definitely a fishman in my eyes he's just referred to as a Kappa because it's Wano so they don't know the outside world similar to how Devil fruits are "magic" within Wano.
