Heads up, the manga and anime only ever call them "cousins" that's it, but there is a Author comment that calls them "distant relatives" so they're probably second or third cousins

There are a lot of ties/connections that don't have an English translation, so 'cousin' is used (which is obviously very confusing for Western audiences). They're not necessarily blood related; they could just share a connection via a family registry. Fruits Basket has the same issue; the Sohmas are a dynastic clan connected via family registry, but because Western countries don't have a similar concept, they keep using "cousin" because it's the closest word translators can find."

Cousin is actually a fairly broad term all things considered, it really can mean basically anything even slightly related

You don't even have to be blood-related to be cousins. For example if your uncle gets a divorce and marries a woman that has a son, her son would technically become your cousin.

Nah, they are 100% OK with first cousin marriage. Functionally, it's really not dangerous, unless your family has a nasty recessive gene floating around. Most of the world doesn't worry about first cousin marriage, it's actually pretty common. In the US we look at it as weird, and honestly I feel like it's weird due to the closeness in my family. But there's nothing extremely concerning about it medically. 10% of all married couples in the world are first cousins.

Hell even a single gen of sibling incest is fine, you only get a marginal of a % chance of minor defects even after 2nd gen of successive sibling incest. The real issues realistically only start to become a problem by gen 10 or w/e, assuming it's successive incest without any other external addition to the pool at any point
