I guess the real question is do you want a custom theme or a child theme or just a minor tweak to what you already have? A true custom theme - (WordPress theme from scratch) is a pretty major endeavor, with the extra added head-ache of always having to maintain it yourself. A child theme is a extra set of theme data added onto another theme, less programming involved and also less prone to breakage when WordPress updates, as the main theme is maintained by someone else. The third option is just a simple CSS override, which can be maintained inside of WordPress, with any added functions coming in via plugins that other people maintain. It's all in what you're looking for. To give you an idea - my current site (smudgemarks-engelwerks.com) is actually a CSS override of ComicPress theme with some extra plugins for functions. ^.^

Speaking as someone who sees a lot of attempted attacks on WordPress -- despite the site I run security for not even having WordPress -- be cautious of custom themes and plugins. In particular, the components I see frequently being attacked are uploadify (also called uploader or just upload) and timthumb (also called just thumbs or thumb). If you find a theme or plugin using either of those, I'd recommend skipping it. If you really like the theme, see if there are patched or replacement versions of the components.
