The 11th Form... i think that Giyu created it himself, because Urokodaki, as a Former Hashira and Master of the Water Breathing Technic, only teaches 10 Water Breathing Forms. Btw, i think it was stated in the Manga, but the " Mother Demon " was almost a Child for herself, when she became the Role of the Mother of Ruis Fake Family. She was the almost Youngest of them all.
