Back on episode 9 there was a flashback of the scene in this episode where Ukitake (Rukia's captain) told her to refrain from helping Kaien. Rukia applied that same situation to Ichigo and let Ichigo handle his own fight against that hollow. Also flashback to the moment where Byakuya one shotted Ichigo in the world of the living, remember when he told Rukia "He does have a resemblance to him" he was referring to Kaien since both him and Ichigo look alike.
