In regards to Alex's death, it's mostly because of the actor issues, but this was also an event that occurs in the books (with a different character, but otherwise largely identical). Whether or not a variation happens in the season that covers that book remains to be seen, especially given that we've already had a few changes.

And to address the timing in the episode, and the positioning of Bull as a potential replacement - The allegations did not come out until after filming had completed, and his writing out had to be inserted at the last minute, with pickups, voice work, and presumably no input from Cas Anvar. Some of the early publicity material even shows Alex sitting in the "empty" arm chair at Avasaralas gathering at the end.
