They sleep on her because of her design literally the only reason. They have to understand this is one piece so character designs don’t determine how strong a character will be most of the time.

Yeah I really don't understand why they value that shit so much. They'd rather have everyone look like Sasuke, than have interesting and varied character design. Like they never give Oda props for that, at least he's trying.

I agree that they may grossly underestimate her in comparison to Kaido. But a lot of that also has to do with how she is handled through out the story, that makes her appear relatively less threatening and incompetent. Most people when they do power rankings generally have her as the weakest Yonko. Whether you agree with that or not is not my point, but just that it's not only RTTV but many people that underrate her for how she is written at times.

I mean she's been shown multiple times to be monstrously strong, almost like a force of nature. The only thing the Straw Hats could do was run away; I think it was pretty clear that she's a big deal. And as far as we know she is the most durable Yonko, we've seen Luffy do more to Kaido at this point in the story than he ever did to Big Mom. And we haven't really seen anything pierce through her skin, yet with Kaido we've seen it multiple times. I think Big Mom's biggest weakness is that she's empty headed, but strength and durability wise it's possible she's the strongest. There's nothing in the writing that contradicts this.

Trust me I know Big Mom is powerful as hell. Even if she fought Roger and lost, he would come out of that battle heavily injured. In fact, he snuck in her territory for the poneglyph rather than going through the hassle of direct conflict, which speaks volumes. Even RTTV was surprised Luffy got any hits on Kaido because they constantly harp on the fact that Big Mom knocked Luffy out of Gear 4. Which shows they acknowledge her strength. No one says that she is not powerful. You can be more powerful than someone and be perceived as less threatening to the reader. Her constant rampages and yelling of "Wedding Cake" while causing a constant ruckus and creating conveniences for the main characters to have the advantage facilitate in lowering her perceived threat. And this amnesia, mother mode, and other plot conveniences make people view her as the bottom of the Yonko, even if that is not true. So all that is to say in RTTV's case they underestimate her, but many people do, and it's not soley bc of character design. It's a shame, but understandable.
