Verdant Vessel

Wondrous item, rare (requires attunement)

The Verdant Vessel seed is a rare and precious gift, produced only a handful of times in the millennia-spanning lifetimes of treants, dryads, and other ancient fey plant beings. Bestowing such a seed is no trivial matter; it signifies an immense debt of gratitude or an unbreakable bond of trust, for within the seed lies a fragment of the fey creature's very soul and essence. This concentrated reservoir of natural energy, when attached to a weapon, allows the vibrant life force within the seed to envelop and infuse the weapon, granting it formidable magical properties. Even in its dormant state, one can feel the pulsating energy of life waiting to burst forth from the seed.

When attached to a weapon, the weapon becomes imbued with the essence of nature, its surface adorned with delicate patterns of vines and leaves. The weapon becomes a magical item that requires attunement unless it already is.

Nature's Embrace. Once per turn, when you hit a creature with the weapon this seed is attached to, you can channel the life force of nature to heal yourself equal to your proficiency bonus.

Awakening

Rebirth Resurgence. When you die, the seed's latent power activates. Vines and roots grow rapidly, enveloping and reshaping your weapon into a new form made entirely of nature's essence. You can spend any number of your hit dice, rolling them and adding your Constitution modifier to each die, regaining hit points equal to the total rolled. Once this feature is activated, the weapon permanently transforms into its new form, and the seed becomes an inseparable part of the weapon. This feature can only be used once.

After the Rebirth Resurgence feature has been activated, the weapon gains a +1 bonus to its attack and damage rolls, unless it is a magic weapon that already has a bonus to those rolls. The healing provided by Nature's Embrace is now equal to twice your proficiency bonus. Additionally, your weapon permanently gains the Nature’s Wrath feature.

Nature's Wrath. As an action, you can invoke the wrath of nature, causing the ground in a 15-foot radius around you to become difficult terrain for your enemies. This radius moves with you. For the next minute, whenever a creature hostile to you moves into or within the affected area for the first time on a turn, vines lash out, and the creature must succeed on a DC 15 Strength saving throw or be restrained until the start of your next turn. Additionally, allies within this area benefit from half cover due to the swirling leaves and petals. While this feature is active, you cannot concentrate on any spells. You can use this feature once, regaining the ability to do so after a long rest.

a little bit of a side note: although the image is that of a sword, I want people to be able to put it on any weapon. Perhaps even on your fist or spell focus (staff, rod, or wand, maybe instead of a weapon attack, you can regain hp after hitting with a spell attack that you cast through the focus). Additionally, my intention of making two separate stats cards is that the Awakening - Rebirth Resurgence is supposed to be a hidden effect that doesn't appear even with identity (maybe just telling you that the seed contains even more energy, dormant inside).

I always love that kind of item, one that either grows with the character or triggers character growth/development~ (and I'm a sucker for a dramatic moment, that's why several of my items has that "crucial decision/moment" type of power XD)

What would happen if the seed were implanted in a person?

I would say the effects is pretty much the same, it affect your natural weapon and unarmed attack. But, as a DM i would probably make it no longer takes up attunement slot after it awaken, since at that point the seed become one with you permanently.
