Bloodstained Bride

Weapon (Shortsword), rare (requires attunement)
Originally this twin blade was forged as a gift and promise from the blacksmith to a noble lady who was madly in love with each other. Unfortunately, the lady was forced by her parents to marry the prince of the neighboring kingdom. Even so, the blacksmith persistently works hard to be worthy of being with the lady. The jealous prince, who feels threatened, kills the blacksmith using the sword before presenting it to the lady at their wedding. The lady who realized what had happened went mad and asked any being that would listen to her to curse everyone that attended the wedding. Something answered and cast a strong curse onto the swords. Wielding the now cursed swords, the bride goes into a murderous rampage, followed by other attendances that become infected by the madness. Initially, the sword handle had a hollow space, but after it was cursed, an eyeball that resembled the bride’s eye filled the hole. When blood is poured onto it, it’ll open up and shed a bloody tear onto the blade, changing its shape into a jagged mess.

You gain a +1 bonus to attack and damage rolls made with this weapon.
Weeping Witness. You can use your bonus action to push the needle on its guard to pierce your fingers. The blood from your hand will then force the eye on its hilt to open, and tears of blood will be poured onto the blade, changing its shape into a jagged mess. While this ability is active, this weapon can’t be disarmed from your hand. At the start of your turn, you take 1d4 piercing damage, but your attack using this weapon deals an additional 3d4 slashing damage. You can use your bonus action again to deactivate this ability.
Red Wedding. While the eye is open, you can cast Crown of Madness without spending a spell slot. When you cast it this way, the spell DC for this spell is 15. After you cast the spell twice, you can’t do so again until you finish a long rest.

I love this kind of weapons that use other materials or energy to reform itself in a more battle oriented form.

Such a clean and simple idea for a blood based weapon. It bleeds you to deal more damage and is able to make people go crazy to go berserk. The lore for this one is grim and poetic, the story really making this one's design shine brighter. One question lingers with me. It was called a "twin blade", but does that mean that there are two Bloodstained Brides meant to be wielded together, or does that just mean it has a blade that matches the Blacksmith's original design somewhere else in the world, never to be its partner?

For this one, I'm leaving it to the DM's discretion. I occasionally make a variant property for a weapon called "twin weapon" which basically allows a character to attune to 2 or more items but count as 1 magic item and only take 1 slot of attunement. But if both items have the same effect, the effect isn't doubled, so you still have 2 uses per long rest for the crown of madness, not 4. But lore-wise, yes, the blacksmith forges two swords, like an engagement ring, one for him and one for the bride. whether player will find a pair, or perhaps only one, and the swords will react/lead the player to its pair, I'll leave that to the DM :D
