Handplug uses Google Forms I believe. Search "Google Forms Add Poll" and there's tutorials on how to make one and then Handplug has the users put their Fanbox name so he can cross-reference it against paying fans so people can't abuse the poll. Not sure if he has some automatic way to do it or if he has to manually check the name against his fanbox fan list.
