When Tony says he'll catch a bullet in the back of the head if anyone finds out he's in therapy, it has nothing to do with it being the 90's or psychiatry being taboo, it's the business he's in. The whole thing about the mafia is keeping your mouth shut, having someone talk about the stuff they do to a therapist isn't something that anyone in that world will take lightly, no matter the year.
