I think they're both the real Buffy. In one dimension Buffy is the Slayer and in another Buffy is a schizophrenic in hospital. Vampire Willow was pulled between dimensions. And Anya talked about places that had nothing but shrimp. The poison could have made Buffy slip back and forth between two different but equally real places.
