So when Ed says his stumps are hurting, he's talking about where what's left of his arm and leg end. The nerves are still active. Arthritic people usually feel more pain when it rains as well. As for Pinako, realize that she's not even related to Ed or Al so what she does for them is truly out of love and not some familial obligation. Regarding human transmutation, it's forbidden for a reason. Even if you try to bring back Al's body or the teacher's organs, you HAVE give up something valued equal in return and as you've seen when Ed and Al tried to bring back their mother, the alchemy god, for lack of a better term, can have a sick sense of what he deems equal. THAT is equivalent exchange.

It's sad that we so often think the worst of ourselves. No one wants to think about this I'm sure but...if the pelvis was male, does that mean it was made to fit Al's when he entered? I suppose the dead coming back to life depends on religious perspective. Good call on Ed being mad at Al.
