I don't quite believe it myself, but I heard a pretty convincing theory once that he really IS a Marshal who came here exactly as was initially presented. That the personnel of the island really were drugging him and the "hallucinated" doctor in the cave really existed. All of his theories about human experimentation were true. This theory posited that we -- the audience -- stopped believing him as soon as somebody told us that he was crazy. Which is exactly what the hallucination-doctor told him would happen: When doctors begin telling everybody else that you're crazy, they disregard everything you say no matter what objections or arguments you try to bring up. Like I said, I'm not really sold on it, but it definitely is an interesting angle. It raises the question of "if somebody like A Doctor HADN'T said 'he's crazy and making things up' would we have believed him?"
