What happened to child-Shigaraki wasn't the Hero's fault, it was the fault of the citizens who saw him and walked past the bloodied child alone on the street and did nothing to help him. Like Shigaraki said "if someone had done something to help him then maybe he would have had a chance at being rehabilitated.
