Playing devils advocate here for a bit: Throat leeches could work as a risk when you're cutting corners. Normally you strain all your water -of course, no need to mention it, because nobody want throat leeches. Buuut... It takes a while. And it requires carrying some extra equipment. So when we're in a hurry, or when a kobold raid has left us without our regular camping equipment, we need to decide if we want to risk the leeches. I feel like it's a more gamey risk than getting the runs from drinking bad water.

I think if you specify that you're going to pay attention to that stuff in *this* part of the map because the Leechity Wilds are so dangerous, you can also get away with it.
