I always thought Ettins came from the idea of conjoined twins. These are not quite so uncommon in farms animals, circus sideshows and anatomical collections as they are in living people (although there is such a skull in, of course, the Hunterian Museum in London).

The idea of the two headed giant brings back memories of Warcraft II and its two headed ogres. They could be upgraded to ogre Magi - one difference was that were ogres' two heads were constantly bickering, the Magi had cooperating heads.
