For samurai in real life Japan, cutting off ones top-knot was a symbol of disgrace. They never come back to it so this is all just speculation/not a spoiler but here I think the meaning is two fold: both that they had lost their honor, and that they were trying to disguise themselves.

I agree with most of the comments on cutting hair = banishment, especially the Fire Nation is largely based on Japan, and samurai who cut off their top knots meant that they cut themselves off of the samurai ways. Off topic, but I have always view the scene where Mulan cutting her hair off (other than disguising as a man) as a symbol of her "abandoning" her daughterly duties to serve as her father's "son".

Yue's death was the first ON-SCREEN death, but if you really look at some of the battles throughout Book 1-- especially that defensive at the Northern Air Temple where Sokka dropped that bomb into the crevice and levelled most of the cliffside-- there were plenty of fatalities, even if they're mostly just implied. That's how they had to do it unfortunately; can't fully depict war on a kid's show. They did an INCREDIBLE job with what they were allowed to get away with, though.
