It may or may not happen to you, it seems random as fuck from my tests.

I'm pretty sure it's because of the new feature allowing to register multiple scripts folders, it must be bugged somehow so the script that fixes the lips on startup sometimes won't work on fresh installation.

I seem to have fixed it but I can't really tell how. I just moved the startup folder from C:\Users\yourusername\AppData\Roaming\Blender Foundation\Blender\3.6\scripts to the desktop and vice versa until at some point it worked again.

Once it's fixed, it seems to hold up so there's that at least.

If someone finds a more reliable fix than brute force and luck, leave it in the comments and I'll edit the post.

so i got the same problem, don't know exactly what you did? Just move the folder away while blender was running, and then moved it back a few times? Well, no luck for me with stuff like this. I can get the script to run manually, which is not that big of a hassle, but it doesn't stick that way, every time i open (the same or a new) file again it isn't active anymore. (i did save first of course). So yeah i hope for a more permanent solution in the future. Hopefully just a bug fix from blender itself. For now i reverted back to 3.5 not only because of this, but i am rendering something at the moment, and i don't know if they changed something in Cycles etc. but i get a slightly different render result (with the same settings) on 3.6. which i of course don't in the middle of rendering a scene.

Try running blender withouth the startup folder then close blender and add the folder back. Honestly though I have no clue as it's completely random. I hate not having a solution but I'm baffled as to why it happens or not. If you run the script directly in the file, you can register it to avoid having to rerun each time. I thought about giving that solution in the post but that creates a lot of potential questions I don't want to deal with.

Haven't found a solution personally - moving the folder sadly doesn't seem to help on my end, no matter how many times the process is repeated. The old Rainbow Mika model can strangely enough have the lip issue resolved, though only for that session (saving and loading again resets the lip to its weird state) by simply clicking the "reset pose" button in your custom UI. Conversely, the UI itself seems to have broken at some other fundamental levels, where you cannot switch from FK to IK and basic functionality like the turning hands to fists sliders etc have simply stopped working. Utterly baffling what the hell Blender has done in this update.

I don't know, apart from the lips thing, what you describe very much looks like you not using the correct UI panel. Mika works fine on my end. As for the lips, clean pose can indeed fix it temporarily, but it's not a solution, especially if we're talking about a file where your character is posed, and it doesn't solve driver errors.
