This episode is a stab through the heart and your reaction really made that apparent again to me. In my opinion, the choice to hide Katsuya's face is due to us being in Tohru's perspective in that scene, and she does not seem to like looking at her father's face, hence why she keeps the picture hidden behind her mom's. I believe she still feels emotionally disconnected from her dad, since she saw him as a force able to take her mom away from her, even if she hates herself for feeling that way. So, in my opinion, that we don't get to see the photo is a reflection of her refusal to look at it. So I think it's more symbolic to her mental state than a reveal, necessarily. Also, as has been mentioned, Kyo reacts to the hat, which has an established relation to his own past trauma and conveniently, has been hidden every other time he's entered her room. So it's his first time seeing it.

Ah, they were never corrected about the fact that the grandfather and his family are Katsuya's family. This has lead to a lot of baseless discussions in previous videos. Katsuya literally cannot be a secret Sohma. The cuckolding theory where he's simply not the biological father still stands, but is unrelated to the picture. Tohru might not be able to remember her father's face, but she has an idea of what he looked like because she knows what her aunt, cousins, and grandfather look like. The face in that picture is going to look similar to her living family at the very least. It literally cannot be a Sohma family member in that picture of her father. Which, as other people pointed out, is not what Kyou was having a reaction to.
