The oversized bags on the squires are kind of a joke callback to the game. The player character can have a companion, and it's pretty common practice to use those companions to carry an obscene amount of your items, so you're not overburdened. Using your companions as pack mules is a handy way to lug a lot of stuff.

In the Fallout Universe, drugs fall under the rather ubiquitous term "chem." Chems range from benign to addictive. Stimpaks are a benign chem, whereas the chem that was used by the Raider in episode 1 could have been one of several addictive chems, but most resembles Jet, which—like Slo-Mo—slows the user's perception of time. It is a different chem than what Coop seems to be taking. Another commenter mentioned that Coop's medication is new for the show, but it seems like he's actually taking the non-addictive chem RadAway; it's meant to quickly reduce the user's radiation levels. Granted, it's administered via IV drip (think: episode 1), but I could see show runners making that change for convenience sake. If that's the case, it's a pretty big departure from the lore of the game, as ghouls are immune to the effects of radiation and don't need to take Rad X or RadAway.

My working theory is that he's lasted so long because of the rad-away and that somehow the extra radiation is tied to going feral faster... in this version anyway...
