Hikaru is kind of an idiot, but you sort of get it, the twins are a little... warped from their isolation growing up, Hikaru struggles with attachments to others but he's improving, Kaoru at least is able to realize it and is able to be more mature and support his brother, you have to remember as well the two of them are only about 15-16 years old, so it makes sense that Hkaru still acts out and doesn't quite know how to handle his feelings. He doesn't have any strong feelings for the girls he flirts with at the host club, but Haruhi is different, so he doesn't know how to emotionally handle seeing Haruhi be close with another guy.
