For the tomb at end, it’s not too clear but they either used too much power and it was open for a little while after they’d left, long enough for what was in there to get out, then it sealed back up after they’d gotten out. This is most likely the answer, as it comes into play next season, and the barriers back up at that time. Some may say, they weren’t strong enough to put it back up, and it stayed down, but grams said it would only be down temporarily, sounding like they were just cracking door open and it’d auto close on its own. And that it was permanently open, but then season 2 with barrier up again would be a season 2 writers mistake. So I believe it was most likely what I’d said at start.

grams' spell is what sealed/closed the tomb back up. once grams died, her spell was broken. in the vampire diaries universe, spells are bound by the witches who cast them. once the witch dies, their spell is broken. so that's how the tomb vampire was able to walk out of the tomb. it was no longer sealed/spelled shut due to grams passing away.
