The beginning of the episode was basically that thing that shows do, where they start with a moment, we as an audience are supposed to wonder how they got to that point and then the rest of the episode explains, well, how they got to that point. Kind of like the 24 hours earlier type story. But with this it's more like Scott's having asthma attacks again, why is that happening, rather than 24 hours earlier. All those moments when it went wide screen and cinematic, were flashbacks to traumatic events. The grandmother drilling into her own head with Lydia, Scott's dog being killed by another dog and Stiles mom's dementia, leading her to believe Stiles was trying to kill her.
