You might be surprised to hear that scene with Simon hasn't been censored from the manga, while he does have more noticeable bruising, it's the same amount of beat up as he was before that attack. Erza's eye thing has been consistent throughout the show and the manga, a lot of the time her hair has been strategically placed against the not-crying eye in a way that you don't notice unless you're looking out for it.
