As someone who started reading the manga after episode 3, I can say that the twists and turns of the story are worth the wait.

Well there's multiple things we don't know yet like everything about the doctor, the man-eaters , the kids in the facility and how the world basically ended. They have been dropping hints though all along. Like the kids having similar abilities to the man-eaters which was confirmed this episode. Also I think the cave scene was probably the girl who died that was missing limbs and everything and when she said oh you made it or whatever was probably the doctor joining her in the afterlife after shooting himself.
