Hey heroes! Thanks for your suggestions and votes this week! Below is the first draft of the Hydra Charm! This item will go through the usual balancing process and be posted here next week following any changes and updates between now and then! Hydra Charm
Wondrous item, very rare (requires attunement)

This golden talisman is made of five twisting hydra heads and hung from a simple gold chain. The talisman has 5 charges and regains 1d4 + 1 expended charges daily at dawn. While wearing the talisman, you can expend 1 of its charges when one of the following triggers occur (no action required) to gain a special reaction. This special reaction is separate from your normal reaction, but follows the same rules and can be used in addition to your normal reaction on a turn.

  • When a creature provokes an opportunity attack, you can use this special reaction to make one melee weapon attack against the triggering creature.
  • When you are forced to make a saving throw against being blinded, charmed, deafened, frightened, stunned, or incapacitated, you can use this special reaction to make the saving throw with advantage.
  • When you take more than 25 damage from a single attack or effect, you can use this special reaction to regain 2d4 + 2 hit points.
  • When your head would be cut off by an effect, such as by a *vorpal sword*, you can use this special reaction to negate the effect.

These are loose item ideas using on the suggestions from yesterday's direction poll. The final product may or may not use the original wording, so these are the core of the items' functionality and flavor.

Ideas

Hydravenom Staff — (Originally Hydra Snake Staff) A staff with 5 snake-like heads that can be used to inject a target with a random poison when hit by it, rolling on a random d6 table of poisonous effects.

Hydra's Bite Hunting Trap — A hunting trap carved with the head of a hydra that grows 4 additional magic heads when stepped on, biting down and grappling the triggering creature and up to 4 other creatures within 15 feet of it until freed.

Multiheaded Pike —  (Originally Hydraskull Pike) A pike made of bone and scales and carved into the form of a vicious hydra. It has 5 charges that can be used once a turn to make an extra attack against a creature within 10 feet of you, summoning a spectral copy of itself to lash out and attack the target.

Charm of the Hydra — A talisman that grants you a second reaction once a turn. This reaction uses a charge from the talisman to either make an opportunity attack, gain advantage on a save against certain conditions, recover some health when you take more than 25 in a single attack, or prevent your head from being cut off.
